Burnout isn’t just about work overload. What feels like exhaustion may actually be a deeper breakdown in how we negotiate purpose, pressure, and trauma.

Burnout is often framed as a problem of overwork. But in this “Vital Signs” interview, mind-body medicine pioneer Dr. James Gordon suggests something more fundamental may be at play: A mismatch between the work people do and the purpose that once motivated them.

He points to a growing reality in fields like health care, where professionals enter with a desire to help, but find themselves constrained—spending limited time with patients and increasingly removed from the human side of their work. Over time, that disconnect can lead not just to frustration, but to a deeper sense of depletion.

What happens next is not just psychological. Gordon describes a sustained physiological response: The body locked in fight-or-flight, with elevated heart rate, rising cortisol, poor sleep, and eventually exhaustion. As this state persists, it can trigger inflammation, anxiety, and withdrawal—creating a cycle that becomes harder to break.

He draws a striking comparison, describing burnout as a form of cumulative trauma—“a wound” to one’s identity.

The response, however, is not purely external. Gordon highlights practical techniques used in settings from post-9/11 recovery to war zones: slow, deep breathing to activate the vagus nerve, movement-based practices to release suppressed stress, and small-group connection to rebuild emotional resilience.
